IMMUNIZATIONS

Immunization requirements are dictated by the Ohio department of health

Kindergarten requirements
    All kindergarten students are required to have up to date immunizations for     Entry to school.
            Up to date immunizations include:
                        5- DTaP (diphtheria, tetanus, and pertussis)
                        4- IPV/ Polio (as of 12/9/09, beginning with the 2010-2011 school
                                                year, the fourth dose must be given after the
                                                fourth birthday)
                        3- Hepatitis B
                        2- MMR (measles, mumps, rubella)
                        1- Varicella (chicken pox)- As of 12/9/09, beginning with the
                                                 2010- 2011 School year, a second dose is required for
                                                 Kindergarten entry
***** All incoming Kindergarten students must have a physical evaluation by a physician or CNP (Nurse practitioner) within one year of the first day of school.

Grade 7
                        1- TDaP (tetanus, diphtheria, and pertussis)- this is a new mandate
                                       from the Ohio Department of health as of 12/9/09, and
                                       will begin with the 2010-2011 school year.
grades 1- 10
                        4-5 DTAP (diphtheria, tetanus, and pertussis)
                        3-4 IPV/ Polio
                        3- Hepatitis B
                        2- MMR (measles, mumps, rubella)

Grades 11-12
                        4-5 DTaP ( diphtheria, tetanus, and pertussis)
                        3-4- IPV/ polio
                        2- MMR (measles, mumps, rubella)

College Vaccine Recommendations:
                        5- DTaP (diphtheria, tetanus, and pertussis)
                        4- IPV/ Polio
                        3- Hepatitis B
                        2- MMR (measles, mumps, rubella)
                        2- Varicella (chicken pox)- If did not have disease
                        Meningitis series
                        2- Hepatitis A
                        Annual seasonal influenza
                        Pneumonia vaccine (if eligible)
