PARTICIPATION FORMS & REQUIREMENTS

1. OHSAA SPORTS PHYSICAL:

All (6) Pages of the physical are due before an athlete can participate in any activity. Missing signatures will result in the form being returned for proper completion. Physical forms are only good for (1) year.

2. EMERGENCY MEDICAL AUTHORIZATION: 

This Form must be filled out and returned to the Coach and Athletic Director prior to beginning participation in an activity. This form contains the emergency phone numbers to call in case your child becomes injured. Each season this form must be re-done.

3. CONCUSSION INFO FORM: 

This Form is an informational form about the new State Law and how it pertains to concussions. It must be signed each school year and returned to the Athletic Office prior to beginning participation in an activity. It does not need to be completed each season, just once per year.

4. ATHLETIC HANDBOOK WAIVER:

The Athletic Handbook Informed Consent Waiver is a student/parent consensual form to agreement of athletic policies and procedures found in the Athletic Handbook. This form is due before the official start date of practices, exact due dates are listed under the section, "Activities & Meetings." This form only needs to be done once per school year.
